All students must be present to pick up the permit as it must be signed in the presence of school personnel.

If you are not a SMHS student you must provide a copy of your birth certificate and proof of residency.
Emailed or faxed copies of applications will be accepted as long as all required information is accompanied with the application. Fax# 508-888-5171
Please read the application and instructions carefully. A current physical OR a signature from the doctor is required.
All students must be present to pick up the permit.
Work permits will be issued Monday through Friday 8:30 a.m. - 1:30 p.m.
The Guidance Office will be closed from June 17- July 1.
