Before any student can tryout or participate in a fall sport at Sandwich Middle High School, the following requirements must be met:

1. Student-athletes must have a valid physical in order to participate in any athletics program at Sandwich Middle High School. Physicals are good for 13 months from the date of the visit. Copies of physicals can be hand-delivered to Athletics, faxed to Athletics at (508) 888-7392, or emailed to athletic trainer, Kelly Spurr, at [email protected]

As school opens, Mrs. Spurr will work with our coaching staff to determine which student-athletes are required to take the IMPACT test this fall. Student-athletes will be notified, asap.

2. A parent/guardian is responsible for filling out this online form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications, and allergies are reported to the Department of Athletics and coaching staff.

3. Student-athletes and parents/guardians must read the Sandwich Middle High School Handbook which can be found HERE

4. Parents/guardians must have completed the FREE, NFHS online concussion course within the past 12 months. The course can be found HERE

IMPORTANT REMINDER: Without proof of a valid physical, student-athletes will not be eligible to tryout for a sport at Sandwich Middle High School. Do not assume that a copy resides with the school nurse. There will be no exceptions made.

Every Sandwich Middle High School program requires a minimum number of participants, usually 8-10. Any program that doesn't meet the minimum number of participants won't be held. Those registrants, of course, will be notified and have an opportunity to choose another program or request a refund.
